Constitutional Signs and Symptoms

Question:  How are you today?
Answer:  My chest hurts and I have been spitting up blood?
Question: What is your name?
Answer: Helen Sparks. The nurse has already asked me these questions.  Couldn't you simply look on the chart for my answers.
Question: I understand that Ms. Sparks however I always like to ask the questions myself so that I can get to know you better. Where are you now?
Answer: In the emergency room. Where do you get these questions.
Question: What time is it?
Answer: I got here at 2:00PM and since you all are soooo slow around here it must be at least 3:30PM by now.
Question: What does the pain in your chest feel like?
Answer: It hurts.
Question: What I meant to ask is was the pain dull, achy, crushing, or sharp?
Answer: Is this a multiple choice test because if it is...  Oh the pain hurts and I guess it is dull and achy.

General comments: Patient is breathing quite rapidly and appears to be in pain on inspiration. She is quite sarcastic and anxious. The patient is very thin and appears slightly malnourished.
